I hereby give permission to Aqua Foundation for Women, Inc. (AFW) and its agents, on behalf of myself, my heirs, personal representatives, and executors, to use any photographs, video or film taken of me for use on AFW’s website, in print material or other media or in other public relations efforts. AFW will use all reasonable efforts to notify me, but notification is not required for the photographs, video or film to be used for public relations purposes. I hereby acknowledge and understand that volunteers are not agents of AFW and, therefore, I am not authorized to make any representation, contract, or commitment on behalf of AFW. I further understand that volunteers are not considered employees of AFW and are not be eligible for, or covered under, any benefit program maintained by AFW, including but not limited to worker's compensation, health insurance, and disability coverage. I understand that volunteers are expected to carrying out their volunteer duties in a mature and responsible manner. I will present myself in a professional manner in public; this includes remaining sober when volunteering at all events. I further understand that AFW has zero tolerance for recreational drugs use during Aqua Girl® or any other AFW-affiliated event. I understand that if I am suspected of being under the influence, I will be asked to leave my volunteer position. I agree that I will not divulge or communicate sensitive or proprietary information regarding AFW or Aqua Girl® acquired by me as a volunteer regarding members, donors, vendors, financial data, long or short term plans, trade secrets and/or any other data of a confidential or sensitive nature, all of which is the sole property of AFW. Volunteers of AFW are acting on their own free will and own risk. In the event of an injury, I personally accept responsibility for the full expense of medical attention as well as other expenses arising out of such injury. I understand that AFW does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness. On behalf of myself, my heirs, personal representatives, and executors, I hereby release and forever discharge, indemnify and hold harmless AFW and its successors and assigns, employees, officers, board members and agents (in this paragraph, altogether “AFW”) from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer services with AFW. This includes any costs, attorney's fees and court costs incurred by AFW in connection with my volunteer services based on damages or injuries, which I may incur in any way while volunteering for AWF. Volunteer understands that this release discharges AFW from any liability or claim with respect to any bodily injury, personal injury, illness, death or property damage that may result from my volunteer services.
